The first is via phone by calling 888-265-8008.



The second is via mail by sending a request to:



America Online 

P.O. BOX 17100 

Jacksonville, FL 32245-7100 



The third is via fax by sending a request to 703-433-7283. 





You will need to provide the full name, phone number, address and handwritten signature of the person who pays for the account, as well as one of the following: 



- The master screen name. 

- The last four digits of the current method of payment.

- The answer to the account security question of the master screen name.
